Jubilee Arts Canada Registration - Ottawa

Consents:

A) I acknowledge that the Participant may be photographed or videotaped while performing activities related to Jubilee Arts and I consent to the use of such photographs/videotapes (the “PV Materials”) by His Highness Prince Aga Khan Shia Imami Ismaili Council for Canada and local Councils (collectively, the “Council”) and agree that the Council shall be the exclusive owners of the PV Materials and hold all copyrights with respect thereto. In entering into this agreement, I am not relying upon any oral or written representations or statements made by the Council and Releases other than what is set forth in this Agreement.  Additionally, I understand that I have the opportunity to seek independent legal advice prior to entering into this agreement.

B) I further agree and give permission to the Council to:

1. send me and/or the Participant communications by email which shall include but not be limited to e-blasts, newsletters, information about competitions;

2. store submitted work of the Participant in its database for the purpose of Jubilee Arts:

3. to use images, videos or other recordings featuring the Participant’s submission to Jubilee Arts

   • on social media platforms such as YouTube, Instagram, Facebook, Snapchat or Twitter;

   • on website platforms such as iicanada.org or the.ismaili website;

   • in newsletters, emails and other communication platforms;

   • on print materials;

   • on digital screens; and

   • to promote Jubilee Arts externally to Jamati activities.

C) I further acknowledge that my name, phone number and email address as well as the name, phone number and email address of the Participant, and other information (such as the Participant’s gender, birth date, address) are being collected by the Council  for the purposes of the Jubilee Arts Canada registration drive and to assist in administering the Jubilee Arts program.  This information will also be used by the Council in creating a database and providing the Council with the ability to advise Participants and their Parents/Guardians of future events hosted by the Council.  I consent to the use of my personal information and that of the Participants by the Council for the foregoing purposes.
