This informative and educational webinar is designed to be a “one stop shop” to answer questions about chronic disease prevention, to help you design a healthy lifestyle plan for yourself and your family, and learn more about the new “Five to Thrive” program.

Dr. Hanif A. Gilani recently completed his medical residency in Internal Medicine and Preventive Medicine. He is dual board certified by the American Board of Internal Medicine (ABIM) and the American Board of Preventive Medicine (ABPM). Over the last few years, he has been working in the domain of disease prevention, and how proper management impacts the lives of his patients. He brings to each patient interaction a person-centered approach.

Dr. Farah Nanji is a registered psychologist who has worked within the mental health for over 20 years, and has seen that there is no separation between mental and physical health. There is a strong inter-connection between how we feel emotionally, how we think and behave, and our physical health. She works with individuals using a holistic lens on health so that they can prevent or reduce the symptoms of chronic mental and physical disease by creating chronic wellness.

Shazia Allibhai is a Senior Clinical Informatics Specialist with Pacific Medical Centers. Through her love for learning, paired with her experience as a cancer survivor, Shazia is committed to maximizing the impact of diet and lifestyle in preventing (and in some cases, even reversing) chronic disease. Her passions include fitness as well as holistic wellness, and as a health coach, she empowers clients along their self-improvement journey.

Moderator Dr. Salima Shamji is a medical doctor with over twenty years of experience in geriatrics, an expert in wellness and resiliency, and an associate professor at the Department of Family Medicine at the University of Ottawa. She is a mindfulness instructor with training in Mindfulness-Based Stress Reduction (MBSR), qigong, and yoga. As an Associate Certified Coach (ACC) through the International Coaching Federation (ICF), Salima can bring all her passions together to empower others to live with intention and thrive. She also serves as the National Director of the Aga Khan Health Board’s Chronic Disease Prevention (CDP) program.
