Collection, Use and Disclosure of Personal InformationThe personal information submitted by you will be collected by the Institutions and used to manage, evaluate and make decisions, assess the key characteristics of families that are served by the program, and to communicate with you regarding other programmes. Such personal information will also be used for the following purposes: (a) to communicate with you and/or deliver non-commercial information, services and programming to you regarding the work of the Institutions ; (b) for Institutional planning and evaluation purposes including, without limitation, to assist with the creation, review and implementation of programs, services and measures intended to benefit the Shia Imami Ismaili Muslim community in Canada and throughout the world (and such information may be shared with other boards and portfolios existing under the Council for Canada and the Institutions for similar purposes); (c) to conduct statistical analysis necessary to further assist the Institutions in fulfilling their mandates; (d) for demographic and community research; (e) for the planning of or response to a natural disaster, emergency event or imminent threat to the Shia Imami Ismaili Muslim community in Canada and throughout the world; and (f) to respond to your comments or questions.
Photo/Video Consent
In submitting registration, I hereby give and grant to the Institutions, the irrevocable right (including, without limitation, all now and hereafter existing common law, statutory and moral rights throughout the world and regardless of whether or not such rights are now known) to use my name, photograph, image and likeness, and voice (the “Attributes”) in any and all publicity purposes, commercial or otherwise, in any and all media without compensation or further notification including, without limitation, the perpetual and unlimited right to reproduce (by electrical transcription, tape or other recording process whether now known or hereafter developed) any materials produced incorporating the Attributes, and the complete and unencumbered right throughout the world, to exhibit, record, reproduce, broadcast, transmit, publish, sell, distribute, perform and use for any purpose, in any manner, by any means and in any medium, whether now known or hereafter developed, all or any part or parts of the matter and things referred to in this paragraph. I acknowledge that I shall not have or claim to have any right, title or interest in or to any materials produced hereunder incorporating the Attributes;
Consent for use of written works, images, presentations, and artworks
Participant(s) agrees to grants rights to the Aga Khan Council for Canada to use participant(s) submitted written works, images, presentations, artworks, and images of such works, in digital form in their promotional campaigns, website, social media sites and other purposes which they deem desirable or necessary in their sole discretion. The participant(s) certifies that no other individual or parties holds copyright interest to the work(s) submitted and that the participant(s) holds all rights to the work(s), and that the license granted herein does not violate any third-party rights or applicable laws.

Release and Waiver of Liability
I fully and forever release, discharge and indemnify His Highness Prince Aga Khan Shia Imami Ismaili Council for Canada, all local Councils, boards and portfolios, volunteers, employees, contractors, representatives, committees, associations, appointed officials, successors, assigns, sponsors of the Program, together with all Program directors, volunteers, staff, coaches, partners, training and medical personnel (collectively, the “Released Parties”) jointly and severally, of and from any and all causes of action, lawsuits, losses, damages, injuries, howsoever occurring, whether by negligence or otherwise (including death), claims, demands, sums, costs, expenses (including legal fees and disbursements), and any other liability of any kind, of or to me or any other person, directly or indirectly arising out of or in connection with my participation in and attendance at the Program.
I AGREE NOT TO initiate any lawsuit, court action or other legal proceeding against the Released Parties, nor join or assist in the prosecution of any claim for money damages which anyone may have, on account of loss, damage, or injury sustained by me or others, howsoever occurring, whether by negligence or otherwise, in connection with my selection of, participation in and attendance at the Program or transportation connected thereto, and I waive any right I may have to do so. This means that I cannot sue to hold the Released Parties responsible for any loss, damage, or injury that I may experience related to the Program.
Authority and Capacity
I have the capacity and legal authority to sign the consents, waivers and releases above and I have read and understood the terms above. The parents/guardians of the Participant(s), by their electronic signature below, also agree to the foregoing on behalf of themselves and of the Participant(s).
