How can Ontario improve health care experiences for both patients and providers while delivering high-value health care and improving population health? Every provider, patient and policy maker wants to ensure excellent care for all – but how exactly do we put quality into action?
Health Quality Ontario, an organization dedicated to advancing the quality care agenda in Ontario, welcomes a call to action specifically targeting patients and caregivers, called Quality Matters: Realizing Excellent Care for All, to learn about experiences with transitioning from hospital to home. Feedback will inform health care quality initiatives across Ontario about how to better support patients as they recover at home after a hospital stay.
