- RENONCIATION ET ACCEPTATION DES RISQUES
Je, le soussigné(e), déclare à mon nom et au nom de mon/mes enfant(s) mineur(s) identifiés ci-dessous (en tant que détenteur l'autorité parentale), le cas échéant, ce qui suit: Je comprends qu’il existe des risques et dangers, connus et inconnus, associés à la présence et à la participation à l’activité proposée, et ce, malgré les règles, l’équipement et la discipline mis en place pour réduire ces risques. Je reconnais et accepte pour moi-même et au nom de mon/mes enfant(s), le cas échéant, ainsi que tous nos héritiers, mandataires, représentants personnels et ayants droit, de renoncer à toute réclamation contre le Conseil Shia Imami Ismaili de Son Altesse le Prince Aga Khan pour le Canada, ses Conseils régionaux et ses institutions affiliées ainsi que leurs administrateurs, dirigeants, employés, agents/mandataires et bénévoles (collectivement appelé les « Organisateurs ») résultant de tout perte, dommage, blessure ou dépense, de quelque nature que ce soit, à moi, mon/mes enfant(s) identifiés ci-dessous ainsi que nos biens respectifs, en raison de notre présence ou de notre participation à l’évènement ci-dessous, que cela résulte d’une négligence des Organisateurs ou autrement, le tout en conformité avec les lois en vigueur au Québec.
- WAIVER AND ACCEPTANCE OF RISKS
I, the undersigned, declare on my behalf and on behalf of my child/children identified below (as the holder of parental authority), if applicable, as follows: I understand that there are risks and dangers, known and unknown, associated with the presence and participation in the proposed activity, and this, despite particular rules, equipment and personal discipline put in place to reduce risk; Furthermore, I, the undersigned, for myself and on behalf of my child/children as well as our heirs, agents, personal representatives and assigns, hereby release the His Highness Prince Aga Khan Shia Imami Ismaili Council for Canada, its regional councils and affiliated institutions as well as their respective directors, officers, employers, agents and volunteers (collectively called the “Organizers”), from any and all claims arising out of any loss, injury, damage or expenses of any kind, to myself or my child/children identified below or our respective property, resulting from participation in, or attendance of the event below, whether arising by reason of the negligence of the Organizers or otherwise, the whole in accordance with the laws in force in Québec.
