The Five to Thrive - Chronic Disease Prevention
The 5 to Thrive are 5 lifestyle areas that have been shown to help improve overall wellbeing. In the coming months, each of the 5 Areas will be highlighted with 5 easy to do actions to help you build healthy habits in each of these areas.
- Healthy Eating - choosing a variety of healthy foods each day.
- Meaningful Movement - adding movement as a regular part of your daily life.
- Stay Positive - building a positive and resilient mindset despite challenging situations.
- Connect with Community - being socially connected to family, friends and your community to support your well-being.
- De-Stress- learning simple strategies to manage the everyday stresses life throws at you.

Why Five to Thrive?
"We are more and more confronted in modern society by non-communicable disease and therefore in the decades ahead we will be concentrating through the Aga Khan Health Network and other medical institutions in dealing with non-communicable diseases. And I refer to diabetes, hypertension, cardiovascular disease, mental and neurological illness, cancer and others. These are the areas where we must concentrate properly, to serve future generations of society."

The prevalence of chronic disease in the Jamat, as in the general population, is growing. Chronic diseases are those conditions that last longer than 1 year and require ongoing medical care or limit everyday activities. In Canada, 44% of adults over the age of 20 experience at least 1 of 10 common chronic conditions. In some cases, members of the community, due to background, are at higher risk, for example, for diabetes or cardiovascular disease. Preventing and treating chronic illness is an important strategy to maintain our overall health and well-being. This is the purpose of Five to Thrive. We encourage everyone to look around this resource site and start taking steps towards better health individually and collectively. Let's work together for a more healthy tomorrow!
