Thank you for expressing an interest in Edmonton Ismaili Soccer League (the “Program”). For the purposes of this Registration, references to “I” or “me” refer to the Participant. 

Please note that this Registration may only be completed by the Participant, their legal guardian or substitute decision maker. At the end of the Registration, you will be asked to confirm that you have legal authority to complete the registration process, including providing consent and waiving liability.
