Required personal and residence information:

• Name, address, telephone number, SIN #, Date of Birth
• Same details for spouse (Name, SIN #, Date of Birth)
• Same details for dependents (Name, SIN #, Date of Birth)
• Property taxes paid or rent paid & name of the landlord
• If living in a nursing home, please obtain a letter from the home detailing the breakdown of the rent and other expenses
• If there has been a change in residence, provide details of all places of residence

Required paperwork:

• Copy of last year’s tax return that you filed
• Your notice of assessment
• T4 slips, including any T4As
• RRSP contribution receipts 
• Charitable donation contribution receipts
• T3s (investment income), T5007
• T5s (interest income)
• Safety deposit box fees receipt/statement
• Medical Expense receipts
• Investment transactions
• University & College students should bring their T2202A tuition certificate (Note: this will need to printed from your University/College website as they are no longer mailed)
